refund form question

refund form question

okay...so i finally got my refund form sent to me. on it, they ask for the credit card information. im not sure i really want to give it to them again, if you know what i mean. is it absolutely necessary to put it to get a refund? has anyone else done it and had no trouble? any help would be great. thanks.

simultaneously submit a complaint to the IFCC.

i put in a complaint to the ifcc long ago, so that is done. so should i put my credit card stuff on this paper?
