I got my DVDs but no bonus disk or lunchbox!?!

I live in Australia and just got my DVDs today, but they didn't send me the lunchbox or the bonus disk!?! 
I ordered in early March so I should be eligible for the lunchbox as well. Anyone else in the same boat? And how do I(we) get the extra stuff we paid for?
