I received my bonus DVD (works fine), my lunchbox (it's in good shape) and my certificate last week -- I live in Birmingham, AL.

It only took 2 years, but I ended up getting it all for free (after getting my CC company to refund the entire charge).

Now just waiting on the bonus disc and (fingers crossed) lunchbox.
